P504S, also known as α-methylacyl coenzyme A racemase (AMACR), is a peroxisomal and mitochondrial enzyme that plays a critical role in bile acid synthesis and the β-oxidation of branched-chain fatty acids. AMACR has been shown to be a highly sensitive and specific marker for prostatic adenocarcinoma. It typically exhibits a strong, granular cytoplasmic staining pattern in malignant prostatic glands, while remaining negative or weakly focal in benign glands. AMACR is frequently used in a triple-stain cocktail with basal cell markers (e.g., p63 and High Molecular Weight Cytokeratin) to increase diagnostic certainty in challenging prostate biopsies.
